A referral is not required for private clients. Department of Veteran Affairs, third party, medicare and work cover clients will need a referral.
The initial assessment will take an hour and is conducted in a private room. At this appointment a thorough assessment of your presenting issue will be undertaken, a medical history gained and questions asked about your current health including diet and fluid intake, exercise levels, mobility and current medications. Other factors that may affect your pelvic floor will be discussed. Subsequent appointments will generally last half an hour.
Pelvic Form Physiotherapy is located at Anna Bay Specialist Rooms, 127 Gan Gan Road, Anna Bay. The Anna Bay Specialist Rooms are situated between the pathology clinic and the chemist at Anna Bay. There is parking at the back of the Specialist Rooms and a car park directly across the road.
Consultation Fees
Standard Consultations Initial Consultation lasting 1 hour $175 Subsequent Consultations
Pessaries, pelvic floor educators, bed wetting alarms, books and other equipment that can aid in your pelvic floor treatment, can be purchased from Pelvic Form Physiotherapy.
Payment Options
Payment is required at the end of the consultation. Payment can be made with cash, debit or credit card. Visa / MasterCard are available. Hicaps is not available at Pelvic Form Physiotherapy.
Enhanced Primary Care Program
I am registered with Medicare for the Enhanced Primary Care Plan. If your GP has decided that you are eligible for a plan he/she will fill out the necessary paperwork. You will need to bring this referral with you, as well as your Medicare card and an Eftpos card so the claim can be processed. This appointment however is not bulk-billed. There will be a significant gap between the cost of treatment and the medicare rebate with the care plan.
